We've had confirmation before that the BA-21 is MMW guided.

We've never seen the domestic variant in service before, but this is clearly it.
What does MMW guided mean? Does that mean there is a small radar in the missile itself that can guide itself to the target? Or does it mean it can be guided by a MMW radar located on a helicopter?

Some websites seem to say it is fire and forget, while Jane's says it was "seen working in conjunction with at least one Z-19A attack helicopter equipped with a mast-mounted millimetre-wave (MMW) radar"

MMW is millimeter wave. Same spectrum used in airport baggage scanners for example.
It works kind of like radar yes but it has a pretty high resolution so the sensor probably can detect the shape of the target better than a long distance radar. You can place the sensors either on the weapon itself, which is what usually happens, or in masts yes.
The most well known example of a missile which uses MMW is the Hellfire missile used in the AH-64 Apache helicopter. Another example would be the Brimstone missile.
Typically a MMW missile is fire and forget. You get a lock on the shape of the target, fire the missile, and its sensor tracks the shape of the target until it reaches it. Or you fire the missile and it seeks targets on its own.
