Altitude is an issue then. MANPADS are short range low altitude weapons. This makes them perfect for attack on low flying slow ground attackers like a helicopter gunship but only suited to attacking airliners during take off or landing. If it was at altitude then it would be to high for a Manpads if it was at at altitude.
Update Boeing has reportedly been invited along with potentially the NTSB to investigate.

Why would Iranian military on alert for US retaliation lock on to a target that was flying in the opposite direction?
Military commanders are well aware of the direct ingress routes for potential attack aircraft. A common tactic is therefore to redirect going out of one’s way to come in from different directions. The Israelis during strikes on Syria often fly out into the med then circle around and launch attacks.
Because of the events of that evening if a US aircraft was in Iranian airspace it would have likely been on a counter strike IE killing IRGC Tels that would have meant it might not have been on a ingress route. However the Issue then would (assuming shoot down and not some other event) have been why wasn’t it apparent to the crew of the hypothetical SAM that they were about to shoot down an innocent party? That’s either a complete failure of the command IE criminal negligence or a technical issue in which the the systems meant to identify a civil aircraft has failed/wasn’t bothered with or a actual intended criminal act.
However I again point out it’s to early. I believe in science on these not reports without confirmation.

There are a lot of weirdness and inconsistencies surrounding this crash.

On the one hand, the Iranians were unusually swift in ruling out all connection between the crash and the strikes against the US on the same night.

On the other hand, it seems awfully convenient that someone was filming the precise piece of featureless night sky at the precise time of the apparent impact. There was no tracking of the missile by the camera, it was just pointed at exactly the right place waiting and ready.

How much work has been done to test the impact footage for fakery? With modern CGI, such footage would be easy to cook up.

No one wants to admit to that kind of event. But Iranian Propaganda would take a blow if they had to admit that they committed there own Vincennes incident.

Lets be clear that the Iranians operate a fully integrated Air Defense System, which is operated by professionals.
It is a very real stretch to imagine that they would not be able to recognise a Jetliner on a scheduled flight, fully transponding and using the official air corridor out of Tehran under the control of Tehran ATC.
If the Iranians were that nervous of retaliation from their missile strikes, surely they would have closed their airspace and grounded all Commercial Air Traffic until such time as they were happy all was clear.

I would be very interested in learning how many aircraft and landed and taken off from Tehran Airport in the intervening time between the Missile strike and the Crash of the Boeing.

So what can I say.
Iranian air defences are modern and operated by professionals and have a good reputation
The last Jet liner shot down in Iran was hit by the Americans
That this is a another crash involving a Boeing
Since when does anyone take the word of the Ukrainians at face value over anything with regard to maintenance?

If I am to make explanations:
IMO, the swiftness of Iranian denial of missile strike can only be due to:
  1. They have counted their missiles and everyone is still on the ground. Very easy to do within some minutes.
  2. They are making a lie that even themselves know is impossible to cover.
For the moment, I simply can not imagine anyone would take the 2nd option.

An Iranian official told the BBC on Friday that there was documentation to prove that the plane had a mechanical issue before take-off. It was not signed off for flying, but Ukrainian airline officials had overruled these objections, the official said, without giving further details.
In the same report, it seems that Ukrainian foreign minister has slightly backtracked his earlier position of "missile shot" to "not to rush to conclusion for the moment".
It is too soon to draw any conclusions on what brought down a Ukrainian passenger plane in Iran on Wednesday, Ukraine's foreign minister has said.
Worth to note is that, the Ukrainian president never hinted any conclusion, it was the foreign minister said "possible missile shot", now he is back stepping. Maybe he was shown the documentation by the Iranians.

Looks like the Iranian missile strike was quite precise, and it was launched from 200-300 km distance.

The USA object visibly lack any point air defence system , or the performance of it is miserable.

The strike wasn't a scud, it was a precise strike weapon.
